It's a figure of speech, not a clinical diagnosis. It's a social statement, not a clinical medical statement. This is consistent with unwanted pregnancy being a social problem, not a medical problem. Saying "we're pregnant" is exactly like saying "we're expecting a child" or "we're going to have a baby" or similar.
