Don't you be silly...Of course there is a chain of command. However, when this malfeasance becomes news, and the Commander in Chief fails to ensure that the person responsible is accountable, then yes he is responsible.
Again no, because just like its not the job of the President to ensure my thieving commander was relieved of command it is not the job of the President to personally oversee the punishment of everyone who does something illegal at their job.
