Why would one expect American single-payer to look like unspecified "other places" and not like what already exists in the U.S.--i.e., Medicare and/or Medicaid, both of which feature heavy involvement of private companies?
my guess that if it's done correctly, it will work pretty much like it does in other places : not perfectly, but not this :
