Although about three times more common than cervical cancer, which strikes about 10,000 Americans and kills about 3,000 Americans per year, oral pharyngeal cancer is still an extremely rare illness, affecting about 28,900 Americans per year, over two-thirds of them male.

I think people have forgotten that we already have a vaccine that is both a anti std ( sexually transmitted disease) and a anticancer vaccine, hepatitis b which has proven its benefit world wide.

An estimated 1 million people die each year from cirrhosis and hepatocellular cancer, which is a direct result of hepatitis B infection. Widespread immunization has led to a dramatic reduction in the occurrence of chronic hepatitis B infection, with such a reduction in liver cancer that the vaccine can be considered the first anticancer vaccine (remember the hepatitis b vaccine is part of routine child vaccinations now.)

as for the topic at hand, smoking or tobacco products still account for 75% of all oral cancers. I think the higher incidence in men is related to the fact that more men use smokeless tobacco. ( from an epidemiology perspective.) I don't see lesbian/bisexual women having higher rates of oral cancer due to HPV.

This doesn't explain the fact that blacks, specifically black males, have the highest incidence rates of oral and pharyngeal cancer.
Men do indeed consume more smokeless tobacco than women; however, blacks do not comprise a large percentage of the smokeless tobacco users.
According to the US Dept of Health and Human Services' latest report on tobacco use in America, the race that uses the most smokeless tobacco is American Indians/Alaskan Natives, followed closely by whites, followed by blacks (a distant third), followed by Asians.
Why then are blacks so particularly hard-hit by oral/pharyngeal cancer, and in fact by practically every type of cancer and communicable illness?

I think it's a combination of diet and lack of access to basic health services, plus... I don't know, there's a theory that some of their vulnerability to hpv might be due to not having been in America as long as some other races, thus having a more difficult time shaking off European variants of HPV16 (the opposite hold true as well; studies have found that white women take longer to clear the African strains of the virus, and therefore are more likely to develop cancer from those strains, since they remain active in the body longer).

"In white women, infections linked to the European variant of the HPV-16 strain persisted for about 17 months, on average, while infections from African variants of the virus lasted only half as long.

In African-American women, by contrast, infections from the European variant lasted 13 months on average. This was a shorter-lasting infection than those caused by African variants of the virus, but only by about 15%.

Exactly why variants of a certain origin are better suited to infect women with ancestors from the same place remains a mystery. It may be because the virus adapts to take advantage of the local human population’s genetic makeup. “It’s kind of an evolutionary dance,” says DeSalle. “That’s the nice way of putting it. Most people call it an arms race.”"

These studies are too small to be definitive, yet something about this idea just rings true to me.

Fully 50% of newly-diagnosed HIV cases in the US are now of African Americans, who comprise only 12% of the general population.
I do not believe it is a matter of "lifestyle choices".
In my experience, no one race is vastly more licentious than any other.
Some races- specifically, black people- are biologically more vulnerable to infection, I believe.
I believe other races are exposed equally as often, but do not succumb to infection as easily.

I'm sure the same holds true for HPV, although of course that's just my personal theory (all of it).

just re-read that study; I had it absolutely back-asswards.
I misunderstood it when I first read it, months ago; I've been laboring under a delusion ever since.
African variants of HPV infect black women longer; European strains infect white women longer. Apparently.

So where does this leave my theory? In shambles, I suppose.

Regardless; there's something, and perhaps it's not geographical. There's some biological factor that causes blacks to be harder hit by nearly every communicable disease (as well as most non-communicable ones) than other races, and I don't believe it's behavioral or lifestyle-related. At least not entirely.

There is no indication, no suggestion whatsoever, that the men who contract oral pharyngeal cancer because of HPVs 16 and 18 in their throats are homosexual men.
