actually it gets caught quite often,rather its one of the hardest things to prove,even if the us govt cracked down on ever accusation,it wouldnt have the manpower to investigate,and bring to trial all or even most of the cases.
of course people go by the innocent until proven guilty,considering how impossible it is to convict the mass amount of suspected voter fraud,most would walk away without any such means of proving it.
infact the us govt doesnt even search for it,local cities search for it during local state and federal elections,while federal authorities tend to pick from a hat.
