Depends on whether or not the civilians flying the white flag are shooting at them, I'd suppose. The established and honorable conventions of warfare don't mean a whole Hell of a lot when one side isn't abiding by them.

Depends on the circumstances, if the enemies surrender doesnt interfere with your objective and initiative then they should be prossesed as prisoners. If the capture of these prisoners will get in the way of the objective they should be killed.

Israel threatens Iran with attack, to which I´m sure would be met by a strong response from the Iranians, and I wonder if the white flag should be respected

You're not fooling anyone, if Iran attacks it'll attack civilians.
It's not capable of projecting power to reach Israel anyway, only missiles and proxy terrorist organizations.
And if Israel attacks Iran it will probably ensure that Iran wouldn't have the means to reach Israel.

I believe no war conventions/laws/treaties allow for the killing of unarmed people trying to surrender. If the current situation makes it impossible to take them prisoner you still would be a murderer if you killed them. ( you could tie them up & leave them with food & water, you could just release them)
