"She/Her" =/= "female of the species", but what social group one belongs to. An effeminate or metro-sexual male who most identifies with an all female pier group will be referred to by that group as, and self identify as, 'one of the girls'. This is no way means that his born sex has changed or that he thinks his body is female. Gender regards identity only.
As it relates to transsexuals:
Gender reassignment surgery is not an attempt to change the born sex from male to female.
It is an attempt to help the individual adjust into the class/group they most identify with.
