I'd say it's time to clean house; but our "softer gentler USA" refuses to address race-crime statistics, illegal alien crime, ghetto crime, and now gun crazed lunatics killing people by the handful in public places like schools, malls, and airports.

    Guns don't kill people. Shooters kill people. I agree - something needs to be done to fix this thug culture in USA and the culture of mass murders for fun and attention. So it seems to me the thing to do is properly place armed military in places where there is a higher expectation that the shootings will happen. Schools, airports, malls, etc.

    or I grew up on RAF bases which were targeted by the IRA and then I joined the Army, hence the word WE.

    Probably a small percentage of the time if you are talking about the major facts; and more so when it is a small supporting piece of information. I would call "TSA being the shooter" a major fact and I hope most news stories don't typically have errors of that magnitude. But reporters love to be the first one to break a story, especially if they are independent and want to put food on their table. You can't be the first to break a story AND wait a few days for all the facts to get sorted out.

    It depends upon the nature of the error, but I would say very frequently.

    Unlike the old days of slow print media, today's digital rush-to-be-the-first is conducive to mistakes.

    In addition, today's 'reporters' seem less focused on accuracy and more focused on speed.

In this day and age though there are always people with their camera phones, so if a journalist doesn't act fast they won't get an exclusive and some video will have gone viral way before it hits AP or Reuters. Alvin Toffler was right, wasn't he. Media made everything an instant global village.
