Lay down at the belly, head away from where the explosion takes place, hide your head with the hands or with something you find, do it fast. The blast could get over you at this place, maybe the heatwave, too, there is some distance to the radiation source.

If you already see a mushroom, than it's pretty late to do something, I guess, however there were pictures of soldiers at nuclear test who were pretty close and survived for the beginning.

That is if the shockwave does not shred you to pieces or the vacuum of does not explode you.

It completely depends on the type of nuke it was. If we're talking something Hiroshima-like, you'd probably have a pretty good chance of survival and probably significant threats of nuclear radiation. I'm not an expert on these things, I'm just going from what I remember from a section in the Imperial War Museum (they have a section and a map of London about it)
